Information regarding the Immunization/Health
Form
Immunization/Health Form is due 3 weeks prior to beginning of classes. Please read and follow the directions on the immunization form and submit it as soon as possible.
Helpful Hints
The Hepatitis B vaccinations series is required for all students who are 18 years of age or younger and pre-med students. This 3 shot vaccination series can be begun & or completed through Student Health Services in collaboration with the DeKalb County Board of Health.
Please remember to mark the month and year of chicken pox disease & or the date of chicken pox vaccination documentation on your health record. NOTE: After age 13 this becomes a 2 shot series. An alternative to the vaccination is to request a “varicella” /chicken pox blood titer drawn at your physician’s office to check for immunity. Documentation of lab results must be attached to the OU Health History and Immunization form. If the results are “no immunity” to chicken pox you will have to begin the vaccination series.
If you have misplaced some of your vaccination records you can request “blood titers” drawn from your physician to check for immunity.
The Meningococcal Disease Information Acknowledgement section must be signed and dated.
Meningitis vaccinations are available at most Pediatrician offices, private health insurance may cover this vaccination. Currently this vaccination is about $110.00 (subject to change), at county health departments.
Failure to
complete the immunization requirements will delay your class
registration and/or attendance.
